Virginia Mason Medical Center (VMMC) has an immediate opening for an Internal Medicine physician to join a group of 12 highly skilled board-certified internists, 5 ARNP's for our regional clinic in Federal Way, WA. Federal Way, the largest of our regional clinics is about 22 miles from downtown Seattle. VMMC offers formalized training in lean methods through our Virginia Mason Production System program. As part of our Team Medicine approach at VMMC, you will collaborate with clinic nurses, medical assistants, administrators, and other colleagues at our neighborhood clinics as well as our downtown Seattle campus. Current call is telephone only and 1:10. Saturday hours will be once monthly for four hours only.

Provides patient specific drug therapy management for patients with complex medication needs. Uses knowledge of differential illness, drug selection, dosing and monitoring to provide treatment plans and achieve therapeutic endpoints.

a. Utilizes appropriate resources to gather data and assess patient care needs. Uses this data, the referring provider's treatment plan and patient provided information to design an optimal therapeutic treatment plan based on defined medical center guidelines and/or other evidence based practices. (BPTF, etc.);

b. Identify, prevent and resolve drug related problems that could lead to adverse outcomes if appropriate action is not taken. Takes corrective action to correct prescribing problems or communicates to providers with recommendations for resolution of drug related problems. Documents in assessments, treatment plans and monitoring according to clinic standards;

c. Provides patients and/or families with focused teaching on safe and effective use of prescribed medications. This is to emphasize key elements and not intended to duplicate information provided by other providers (e.g. Retail pharmacist).

Provides population based medication management for certain disease or drug areas. (e.g. anticoagulation, HASP, Diabetes, hypertension, Lipid and Thyroid)

a. Manage the panel of anticoagulation patients for the specific clinic.

b. Manage the high risk, complex patients (medication intensive) patients enrolled in specific disease management programs (HSPC, Diabetes) or assigned via the clinic registry.

c. Maintains all site specific materials for prescriptive authority protocols (current protocols, authorized provider list, authorizing physician signature pages and all Board of Pharmacy correspondence.

Collaboration, delegation, supervision:

a. Forms partnerships with all members of the interdisciplinary team;

b. Communicates with coworkers and other departments in a professional manner; shares appropriate information with all members of the health care team, including assistive/support personnel, in a manner that is appropriate and timely for the patient's condition;

Advanced training (residency or fellowship) in pharmacy practice or >3 years experience in an outpatient setting required. Excellent drug management skills, ability to work collaboratively in team setting, experience with using electronic medical records, and demonstrated expertise in prescribing drug therapy under protocol also required. Must meet requirements for ancillary provider credentialing at Virginia Mason.

Experience in managing a broad range of complex, medication intensive patients in an ambulatory setting highly desirable. The clinic is open six days a week, and coverage of the Saturday clinic from 9:00 am – 2:00 pm is covered currently by the physician on-call that week. The clinic has extensive access for its patients and offers its providers a great extent of flexibility to meet their own worklife needs.

The Ophthalmic Assistant II, Certified (i.e., JCAHPO COA) provides assistance to Ophthalmologists/Optometrists by performing clerical and clinical duties required to most effectively handle the day-to-day functioning of their practices. This job performs all duties of the Ophthalmic Assistant I, Certified, as well as end-point refractions and higher-level ophthalmic testing.

1. Greets and rooms patient. Completes patient work-up, including collection of history and chart information such as chief complaint, medications, and allergies. Performs testing of vision and neutralizing of spectacles. Ensures the fee sheet is properly completed and ready for data entry. Enters information into Cerner in accordance with VM standard process. Takes intraocular pressure by applanation or Tonopen, and performs refractometry. May perform additional tests including, but not limited to: A- or B-scan calculations, Visual Fields, Pachymetry, Corneal Topography, Heidelberg Retina Tomography (HRT) and Optical Coherence Tomography (OCT). May assist physician with in-office procedures, as appropriate and in accordance with incumbent's training. Administers ocular medications as prescribed by the physician. Educates patients as necessary or requested.

2. Provides patient counseling regarding proper use of medication. As needed, distributes approved pre- and post-op guidelines.

3. Performs telephone triage to assess urgent versus emergent needs and creates schedules to meet the needs of the patient. Works with e-call physician's teams to work in patients. Does general scheduling of patients and handling of phone messages.

4. Restocks and cleans exam rooms after patient care. Conducts area sweeps during and at end of day to ensure that team area is always at a level 4 per 5S guidelines. Closes rooms at end of patient schedule.

At least one year experience in an ophthalmology practice, and completion of an approved training program and certification by the Joint Commission of Allied Health Personnel in Ophthalmology (JCAHPO) as a Certified Ophthalmic Assistant. Working knowledge of anatomy, pharmacology, medical terminology, sterilization techniques, and basic eye care. Understanding of ophthalmic testing and the use of ophthalmic instruments and equipment. Skills in obtaining intraocular pressures through applanation tonometry and use of the Tonopen; performing end-point refraction and neutralizing spectacles; and using a slit lamp required. Basic skills in operating a computer and office equipment. Strong interpersonal and customer relations skills; strong communication skills; and organizational skills required. Strong leadership skills required. Ability to make chart documentation with entry into Cerner.
